Right of Access Request
Right of Access Request Form SA1
Application for access to your personal data held on West Yorkshire Police information systems
A data has the right of access under the General Data Protection Regulations (GDPR) Part 3 Law Enforcement Article 45 and under the GDPR Part 2 General Processing Article 15.
Your Right of Access
Subject to certain exemptions, you have a right to be told whether West Yorkshire Police holds any information about you (your ‘personal data’) and a right to be provided with a copy of that personal data within a month.
If you wish to exercise those rights and to assist with processing please complete this form carefully and follow the instructions regarding proof of identity, and ways to return the form. Alternatively, please contact West Yorkshire Information Management via [email protected].
The General Data Protection Regulations mean that in certain circumstances West Yorkshire Police may decide not to provide you with some personal data. For example, we will not provide personal data if we feel releasing it to you would be likely to prejudice policing purposes, and we may not provide you with information that identifies other individuals.

Proof of Identity
West Yorkshire Police needs to be satisfied that you are who you say you are. Consequently you need to provide evidence of your identity and address by supplying copies of at least two official documents which between them provide sufficient information to prove your name, date of birth, current address and signature. If you require these documents returning then please include a stamped addressed envelope.
You will be required to send identification documents to both ACRO and West Yorkshire Police with your applications. Please ensure that you have noted on the SA1A application that you have submitted both requests.
Requests for Information for Employment Purposes
West Yorkshire Police do not provide PNC (Police National Computer) disclosure for employment vetting services. If you require a disclosure for UK employment purposes please contact
Disclosure and Barring Service (DBS) https://www.gov.uk/government/organisations/disclosure-and-barring-service
Disclosure Scotland https://www.mygov.scot/organisations/disclosure-scotland, or
Access Northern Irelandhttps://www.nidirect.gov.uk/campaigns/accessni-criminal-record-checks as appropriate.
The current procedure whereby companies require individuals to exercise their rights under GDPR Right of Access provisions, as a form of vetting, has been considered to be unlawful for many years and is against the spirit of the legislation. It will be a criminal offence for the employer to ask individuals, as a condition of employment, to go through this process.
Third Party Application
We can only supply your personal data to you. Should you be making an application on behalf of any other person (third party), please ensure you enclose an original letter of authorisation from that individual (the person who the information is about). Please also ensure that this individual has signed the application form and provided their identification documents.
